Permission and Release of Coaching Session: 
Recording Authorisation

As part of my deep commitment to becoming a professional ADHD coach, I am pursuing quality education and credentialing from my coach training program. I am completing my training through the ADD Coach Academy (ADDCA), and will be accredited by ADDCA, along with higher coach credentialing organisations, such as the International Coaching Federation (ICF) and the Professional Association for ADHD Coaches (PAAC). For the purposes of educational, professional development and/or credentialing, I am required to produce audio or video recordings of coaching sessions with my clients with written transcripts of the sessions.

‌Any student, mentor, trainer, or assessor who reviews the recorded sessions will handle all information with confidentiality under the ADDCA, ICF, and PAAC codes of ethical conduct.  

‌I am requesting your permission to record our coaching sessions and submit the recording and transcript for my educational growth, professional development, and/or credentialing purposes. I agree to adhere to and be subject to the same ADDCA, ICF, and PAAC codes of ethical conduct.

‌This form, when completed and signed, authorises Caroline Beresford, your coach, to record and release coaching sessions with you.

‌Caroline Beresford, your coach, will make it clear when the sessions are being recorded.

‌This authorisation shall remain in effect until 24th October 2023.

‌You have the right to revoke this authorisation in writing at any time by sending such written notification to Caroline Beresford at [email protected]
